I, the undersigned applicant, authorize McConnell Memorial Baptist Church through its independent contractor, Trusted Employee, Inc., to procure background information (also known as a “consumer report” or “investigative consumer report”) about me as a part of my application for employment or in a volunteer ministry position. This background check may contain information regarding my references, character, criminal history, social security verification, motor vehicle records (“driving” records), verification of my education or employment history, or other background checks. Credit history will only be requested where such information is related to the duties and responsibilities of the position for which I am applying. I understand that, at the discretion of the Lead Pastor, periodic background investigations may be performed on paid employees or volunteers. I understand that I am entitled to a complete copy of any background information of which I am the subject upon my request to McConnell Memorial Baptist Church, if such is made in writing within 90 days from the date it was produced. I also understand that I may receive a written summary of my rights under the Fair Credit Reporting Act. *
